You need to go to main social insurance in your city on Cyprus and fill the forms (requesting sending  forms to social insurance centers in your country) You will need to know your cyprus social insurance number and your social insurance number you have in your country. You will also need the data like: names and adresses of your employers on cyprus, dates when you where employed with each of them, your adress in your country and address of  social insurance institution in your country where the forms should be send.
